Health Fund Claiming Guidelines and Instructions
To ensure a smooth process when claiming benefits for your contact lenses through your health insurance fund, please adhere to the following guidelines:
-
Verify Your Coverage: Before submitting your claim, ensure you have extras optical cover included in your health insurance policy. Note that hospital cover alone does not include optical services.
-
Valid Prescription Requirement: You must have a valid and current contact lens prescription from a qualified optometrist. Order only contact lenses prescribed explicitly for health fund insurance claims. Upload your prescription to your profile under My Profile -> Prescriptions -> Upload Prescription.
-
Name Matching: To avoid complications during the claims process, ensure the name on your contact lens order matches exactly with the name registered on your health insurance policy. Any name discrepancies could result in the rejection of your claim.
-
Non-Prescription/Plano Lenses Exclusion: Non-prescription or plano contact lenses, considered cosmetic products, are not eligible for health fund claims. You can only claim benefits for contact lenses prescribed for vision correction.
-
